- Form DS-11 which is the official application for U.S. Passport.
- Evidence of U.S. Citizenship and Photocopy
- Valid Identification and Photocopy
- Application Fees
- Passport Book: $110.00 Execution Fee: $35.00
- Passport Book & Card: $140.00 Execution Fee: $35.00
